Coloring Contest

Coloring Contest February 1st-28th, 2013 Submit your entry on a 8.5 x 11 paper, using crayons, colored pencils, paint, ect. You may draw whatever you like as long as there is ONE dental item (toothbrush,floss, dentist, assistants, teeth, ect.) One entry per patient please. Please include your name, age, and phone number on the back of your entry. Prizes will be awarded for age categories: under 6, 7-10, 11-13, 13+ Submit your entry to our office in CDA Monday-Thursday/Sandpoint office Tuesdays, or mail to 1717 Lincoln Way, CDA, ID 83814
